What to Expect at Your First Physical Therapy Visit

Booking your first physical therapy appointment can feel like a big step, especially if you've never been to PT before. You might be wondering what will happen, what you should wear, or whether it's going to be painful. Those feelings are completely normal, and I want to put your mind at ease.

At JointWorks PT, your first visit is designed to be thorough, comfortable, and zero pressure. Research shows that a strong relationship between patient and therapist leads to better pain outcomes. [1] By the end of the hour, you'll understand what's going on with your body, what the plan is, and exactly what to expect moving forward. Here's a step-by-step walk through your first appointment at my Northbridge or Ashland location.

Before You Arrive

Start With a Free Consultation (Optional)

If you're not sure whether physical therapy is right for you, I offer a free consultation. This is a short, no-pressure conversation where you can tell me what's going on, ask any questions, and get my honest opinion on whether PT makes sense for your situation. You can book this online or by phone. There's no commitment involved.

Paperwork

When you book your first full appointment, you'll receive a link to complete your intake forms online before your visit. This covers your medical history, current symptoms, medications, and your goals for treatment. Filling this out ahead of time means we can use every minute of your appointment for hands-on care instead of clipboard work.

What to Wear

Wear comfortable clothes that let you move and let me see the area we're working on. Here are some general guidelines:

  • Back, hip, or knee pain: Shorts or athletic pants and a t-shirt
  • Shoulder or neck pain: A tank top or loose t-shirt that lets me access your shoulder
  • Ankle or foot pain: Shorts so I can see how your leg moves, and bring the shoes you wear most often

You don't need any special gear. Just think "gym clothes" and you'll be fine.

What to Bring

  • Any imaging results you have (X-rays, MRI reports) if available
  • A list of medications you're currently taking (if you didn't include it in your paperwork)
  • Your insurance card if you plan to submit for out-of-network reimbursement
  • Questions. Write them down so you don't forget

What Happens During Your First Visit

Your first appointment at JointWorks PT is a full 60-minute session, just you and me. No aides, no sharing time with other patients, no rushing. Here's what we'll cover:

Step 1: We Talk

The appointment starts with a conversation. I want to hear your story in your own words. When did the pain start? What makes it better or worse? What have you already tried? What are you hoping to get back to? This isn't a checkbox interview. I'm listening closely because the details of your story often point to what's going on even before I put my hands on you. Studies show that how your therapist communicates directly affects your results. [4]

Step 2: The Physical Evaluation

Next, I'll do a thorough hands-on assessment. Depending on your issue, this might include:

  • Watching how you move (walking, squatting, reaching, bending)
  • Testing your range of motion in the affected area
  • Checking your strength in specific muscles
  • Hands-on tests to assess your joints, muscles, and nerves
  • Looking at your posture and how different body parts work together

I'll explain what I'm doing and why as we go. If anything is uncomfortable, just tell me. This evaluation is about gathering information, not pushing through pain.

Step 3: I Explain What I've Found

Once the evaluation is done, I'll sit down with you and explain what I think is going on. No medical jargon, no confusing terms. I'll tell you in plain language what's causing your pain, why it's happening, and what we can do about it. [3] I'll answer every question you have. You should leave your first visit understanding your body better than when you walked in.

Step 4: We Build Your Plan Together

Your treatment plan isn't something I hand down from on high. We build it together based on your goals, your schedule, and your life. I'll recommend how many sessions I think you'll need, how often we should meet, and what you can start doing on your own right away. I'll also give you an honest assessment of what to expect in terms of timeline and results.

Step 5: We Start Treatment

In most cases, we'll begin some hands-on treatment during your first visit. This might include manual therapy to improve joint motion, soft tissue work to release tight muscles, or a few exercises to get you started. Many patients feel noticeably better after just this first session.

After Your First Visit

You'll leave with a clear understanding of:

  • What's causing your pain or limitation
  • The plan to address it
  • Exercises or strategies to use at home right away
  • How many visits we anticipate needing
  • What to expect at your next appointment

I'll also check in on how you're feeling over the following days. It's normal to feel a little sore after your first visit, similar to how you might feel after a good workout. That usually settles within 24 to 48 hours.

Common Questions About the First Visit

Do I need a referral from my doctor?

No. In Massachusetts, you have direct access to physical therapy. That means you can come straight to JointWorks PT without a referral. [2] Research shows direct access leads to fewer visits, lower costs, and high patient satisfaction. If your situation needs further medical workup, I'll coordinate with your doctor directly.

Will it hurt?

The evaluation itself shouldn't be painful. Some of the tests might reproduce your symptoms briefly so I can understand what's going on, but I'll always communicate with you and adjust as needed. Treatment is guided by your comfort level.

How much does it cost?

JointWorks PT is a cash-based practice, which means you pay directly for each session. This gives you longer appointments, better care, and no surprise bills. I'm upfront about pricing from the start, and I also offer packages that reduce the per-session cost. Many patients use HSA or FSA funds, and I provide superbills for out-of-network reimbursement.

What if I'm nervous?

That's completely okay. Many of my patients tell me they were nervous before their first visit and felt completely at ease within the first few minutes. My job is to make you feel comfortable, listened to, and confident in the plan. If the free consultation helps ease that anxiety, start there. It's a quick, low-key way to get to know me and ask any questions before committing to a full visit.
